A centralized wiki eliminates these issues while still allowing you to keep a historical record if you need to recover old documentation. These wikis can be set up to be local and secure on your own internal network or hosted on a provider for easy access anywhere. MkDocs is best for documentation created by a single person or small team. It can create exports as web pages or PDFs. I primarily used this tool for creating my personal wiki before the need to share the information to the public. DokuWiki is best for documentation created by large teams or for documentation that requires access control. If you have information that needs to only be accessible to specific users, the ACL rules allow fine grained control over who can view, edit, or create pages.
